I was having this problem after changing Doom3's quality levels. It seems Doom3 did not properly reset this setting after returning to medium from low quality. The value for s_MaxSoundsPerShader was set to 1, so I set it to 0, restarted, and jumped around on some things (erm, yeah) and it seemed to fix it.<br />
